Exacerbation of Repetitive Falls Due to Atonic Seizures Following Perampanel Administration

A 47-year-old man presented with tonic-clonic seizures characterized by convulsions. He repeatedly exhibited despite treatment four anti-seizure medications. During the titration process of perampanel (PER), paradoxically increased in intensity and frequency, resulting trauma. Video electroencephalogram monitoring revealed interictal rapid rhythms generalized spikes documented atonic seizures. Thus, patient was diagnosed Lennox-Gastaut syndrome. Upon discontinuation PER, patient’s falls improved, probably suggesting a paradoxical effect PER. non-competitive antagonist selective for AMPA (α-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid) receptors may have caused weakness delayed recovery from prolonged atonia that injuries.
